Serious Case Reviews

 
Following the unexpected death of a child we may commission a Serious Case Review in accordance with the guidance contained in the HM Government document 'Working Together to Safeguard Children 2006'.
 
A Serious Case Review may also be commissioned where a child has 'sustained a potentially life-threatening injury through abuse or neglect', to identify whether lessons should be learned by the agencies involved with the family.
 
Serious Case Reviews will result in the publication of an Executive Summary report.

Below you will find Executive Summary reports of Serious Case Reviews that have been commissioned by the Sheffield Safeguarding Children Board (formerly the Area Child Protection Committee, ACPC).
 
Executive Summary reports will be available on this website for four years following the date of publication.
