Serious Case Reviews

 
Following the unexpected death of a child we may commission a Serious Case Review in accordance with the guidance contained in the HM Government document 'Working Together to Safeguard Children 1999 '.
 
A Serious Case Review may also be commissioned where a child has 'sustained a potentially life-threatening injury through abuse or neglect', to identify whether lessons should be learned by the agencies involved with the family.
 
Serious Case Reviews will result in the publication of an Executive Summary report.
