Child Death Processes

 
As from April 2008, Sheffield Safeguarding Children Board has to have specific procedures in place when a child, who is under the age of 18, dies. The key requirements are that we:
 
  • have a rapid response by a group of key professionals who come together for the purpose of enquiring into and evaluating the unexpected death of a child
  • have a overview panel of key professionals who come together to review the deaths of all children resident in Sheffield.

  • the procedures for the operation of the Sheffield Rapid Response Team 
  • the procedures for the operation of the Sheffield Child Death Overview Panel
  • initial notification of child death form, to be completed for each child who dies 
  • information leaflet for staff
